Supplementation with P-5-P, Pyridoxal-5'- Phosphate, may be useful for those unable to produce it efficiently from pyridoxine in the liver. P-5-P, the "activated" coenzyme form of B6 is extensively involved in the metabolism of amino acids, nucleic acids and lipids. In the 1950's, researchers found deficiencies of B6 apparently related to increased use of prescription drugs, as well as the use of artificial preservatives, additives and colorings in our food. Excessive alcohol use, protein intake or contraceptive use may indicate a greater need for vitamin B6.Observations during clinical treatment of patients who need vitamin B-6 supplementation have shown that when P-5-P is indicated, it is 10 times more effective than pyridoxine HCl.Decreased plasma levels of P-5-P have been shown in the following conditions:Immunological Disorders, Renal Disorders, Atherosclerosis, Hepatic Biliary Disorders, Pregnancy/Lactation and Cancer.In addition, patients with cirrhosis show an increase in alkaline phosphatase and degradation of P-5-P. Intestinal absorption of P-5-P is decreased in patients with celiac disease. The following drugs have been reported to decrease plasma levels and increase metabolic requirements of P-5-P: Oral Contraceptives ,Hydrazines ,Cycloserine , L-Dopaand Penicillamine . Supplementation with Pyridoxal-5’-Phosphate is indicated for patients with amino acid deficiencies and intestinal malabsorption as well as for patients with increased need for vitamin B-6 due to toxicity, infections or metabolic impairments.Since there are many causes for vitamin B-6 deficiency, patients who do not respond to vitamin B-6 supplementation with pyridoxine HCl may benefit from Pyridoxal-5'- Phosphate supplementation.
